Be Recruitment Group is partnering with a health and community care provider to recruit a Care Partner. This opportunity would suit a Registered Nurse who enjoys building trusted relationships and making a practical difference in people's daily lives.

Reporting to the Care Services Manager, you will support clients remain in their own homes, delivering care that respects their choices, promotes independence and responds to their individual needs.

Qualifications

• Registered nurse with current AHPRA registration.
• Commitment to client dignity, choice and independence.
• Clear observation and escalation of risks.
• Strong communication, organisation and documentation skills.
• Willingness to obtain National Police Check and NDIS Worker Screening.

Responsabilités

• Deliver personal care and in-home support per care plans.
• Assist daily living activities: mobility, hygiene, nutrition and wellbeing.
• Monitor health changes and escalate concerns promptly.
• Communicate with families, coordinators and professionals.
• Maintain accurate care records and incident reports.
• Uphold infection control, manual handling and safeguarding standards.
• Use digital systems to record information and contribute ideas.

About the opportunity

The role

• Deliver personal care and in-home support in line with individual care plans.
• Support daily living activities, including mobility, personal hygiene, nutrition and wellbeing.
• Monitor changes in clients' health, behaviour or home environment and escalation concerns promptly.
• Communicate with families, care coordinators and other professionals to support continuity of care.
• Maintain accurate care records, observations and incident reports.
• Uphold infection control, manual handling and safeguarding standards.
• Use digital systems effectively and contribute ideas to improve care delivery.

About you

You are a compassionate and reliable Registered Nurse who takes pride in providing respectful, individualised care. You communicate clearly, recognise when something needs attention and work collaboratively to achieve positive outcomes for clients.

You will bring:

• Current registration as a Registered Nurse with AHPRA.
• A commitment to client dignity, choice and independence.
• Sound observation skills and the judgement to recognise and escalation risks.
• Strong communication, organisation and documentation skills.
• Confidence using digital systems to record and share information.
• A current National Police Check.
• A NDIS Worker Screening Clearance, or willingness to obtain one.
• A valid driver's licence and Australian working rights.
• The ability to meet relevant immunisation requirements and obtain a Working with Children Check where applicable.
