Job summary

Silverchain is seeking a Care Partner to support older Australians in the Central Coast region. You will conduct client assessments, develop personalised care plans and coordinate services to keep clients safe, independent and connected at home.

The role combines community-based client visits with the flexibility to work autonomously from home within a 6-month fixed-term contract. You’ll join a supportive team and work with clinical, scheduling and service delivery staff to deliver outcomes that

Qualifications

• Experience in community-based case management.
• Experience with government-funded aged care services is advantageous.
• Strong communication and relationship-building skills.
• Empathy, compassion and client-centred approach.
• Excellent organisational and time management skills.
• Ability to work across multidisciplinary teams.
• Current driver’s licence and a reliable vehicle with appropriate insurance.

Responsibilities

• Manage a caseload of older Australians receiving aged care services at home.
• Conduct client assessments and develop personalised Care Plans.
• Monitor client budgets in line with program requirements.
• Coordinate services to ensure timely support.
• Support new clients through onboarding and ongoing service delivery.
• Build relationships with clients, families and stakeholders.
• Collaborate with clinical and scheduling teams to deliver coordinated care.
• Advocate for client needs and positive health outcomes.
