Job summary

Mounties Group in Sydney's Hills District is seeking a dedicated Care Partner to join our Mounties Care Home team. This full-time role focuses on coordinating client care, delivering services in the home, and guiding carers to ensure high-quality care for clients in the community.

The ideal candidate will have 2–3 years in a similar role, CPR/First Aid, and strong knowledge of Aged Care standards. You will work closely with clients, families, and service providers to maintain excellent care

Qualifications

• Minimum 2–3 years’ experience in a similar role.
• Current clear National Police Check.
• Current First Aid Certificate, including CPR.
• Demonstrated knowledge within the Aged Care sector.
• Demonstrated care coordination skills and ability to work effectively with clients, family members and services providers.
• Previous experience managing Home Care Packages.
• Experience navigating the My Aged Care Portal.
• Strong understanding of the Aged Care Quality Standards.
• NDIS knowledge desirable.
• Prior experience with CRM and electronic rostering system.
• Proficient computer skills, including Microsoft Office.
• Effective verbal and written communication skills.
• Excellent time management skills, with the ability to multitask.
• Positive and professional can-do attitude.

Responsibilities

• Develop and coordinate client care plans.
• Prepare individual client budgets.
• Assist clients to access My Aged Care.
• Visit clients in the community.
• Meet with potential client leads and sign them up to our services.
• Participate in after hours on‑call roster arrangement.
• Maintain consistency and continuity in the clients' care regime.
• Liaise with staff, clients, families and Healthcare providers as required.
• Maintain client confidentiality.
