Job summary

Mounties Care Home is seeking a dedicated Care Partner to join our Hills District team on a full-time basis. You will coordinate and deliver in-home care and services to help clients stay independent at home and in the community.

As part of a supportive team, you will develop care plans, manage budgets, assist clients with My Aged Care, and ensure continuity of high-quality care. Strong coordination and communication are essential.

Qualifications

• 2-3 years' experience in a similar role
• Current clear National Police Check
• Current First Aid Certificate, including CPR
• Demonstrated knowledge within the Aged Care sector
• Demonstrated care coordination skills and ability to work effectively with clients, family members and services providers
• Previous experience managing Home Care Packages
• Experience navigating the My Aged Care Portal
• Strong understanding of the Aged Care Quality Standards
• Prior experience with CRM and electronic rostering system
• Effective verbal and written communication skills
• Excellent time management skills, with the ability to multitask
• Positive and professional can-do attitude

Responsibilities

ABOUT THE ROLE

Mounties Care Home care is searching for a dedicatedCare Partner to join our team on afull-time within the Hills District, NSW!

The successful candidatewill work with our clients to assist them in remaining as independent as possible in their home and community. You will coordinate the delivery of care and services to our clients and have oversight over our team of Carers ensuring that quality services are delivered.

More specifically, some of your key duties will include:

• Develop and coordinate client care plans
• Prepare individual client budgets
• Assist clients to access My Aged Care
• Visit our clients in the community
• Meeting with potential client leads and 'signing them up' to our services

ABOUT YOU
