Job summary

Melbar in Sydney is seeking a full-time Care Partner to join our in-home aged care team. You will assess, coordinate, and manage care services for our support-at-home clients with a holistic approach to clinical and nursing care.

The role emphasizes person-centred plans, collaboration with nurses and allied health professionals, and adherence to aged care standards. Mandarin language skills and a valid driver’s license are essential for this position.

Qualifications

Certificate III in Ageing or Health Services Assistance required.
Full-time working rights in Australia.
Mandarin language ability is essential.
Driver's license required.

Responsibilities

Conduct care assessments, develop person-centred care plans and individualised budgets based on participants' needs, goals and preferences.
Coordinate and manage services with Service Coordinators, Registered Nurses, allied health professionals and other relevant providers.
Monitor and review participants' wellbeing, care needs, service delivery and outcomes, and update care plans as required.
Monitor individualised budgets, service utilisation and expenditure to ensure appropriate use of available funding.
Identify, assess and manage risks, incidents, complaints and safeguarding concerns, and escalate clinical issues to relevant professionals.
Maintain accurate participant records, provide ongoing support and education, and ensure compliance with aged care legislation, quality standards and organisational policies.
