Job summary
Melbar is seeking an energetic Full-time Care Partner to join our team in Sydney. You will assess and coordinate in-home aged care services, develop personalised care plans, and work with Nurses and allied health professionals to ensure high-quality client outcomes.
The role requires Mandarin language proficiency, a Certificate III in Ageing/Health Services, a valid driver’s licence, and previous community care or nursing experience. Hybrid work is available, with up to 3 days at home.
Qualifications
- Mandarin language ability is essential.
- Certificate III in Ageing/Health Services required.
- Valid Australian driver's licence to operate a vehicle.
Responsibilities
- Conduct care assessments and develop client-centred care plans and budgets.
- Coordinate services with Service Coordinators, RNs, and allied health professionals.
- Monitor wellbeing, service delivery and update care plans as required.
- Oversee budgets and service utilisation to ensure funds are used appropriately.
- Identify, assess and manage risks, incidents, complaints and safeguarding concerns.
