Job summary

Leora Healthcare is seeking a Care Partner to join our South East Melbourne team. In this community-based role, you’ll work with clients, families and care teams to coordinate personalised support that promotes independence and wellbeing.

Starting January 2027, you’ll assess needs, develop care plans, and advocate for clients in their homes and local communities. You’ll ensure services align with goals, funding requirements and high-quality standards.

Qualifications

• Qualification in allied health, aged care, disability or related community field.
• Experience in community engagement, including networking and stakeholder engagement.
• Strong stakeholder management and relationship-building skills.
• Excellent verbal, written and presentation communication.
• Strong organisation and decision-making skills, especially in fast-paced environments.
• Based in Southeast Melbourne with a valid Australian driver’s licence.
• A valid NDIS Worker Screening Check, or willingness to obtain one.

Responsibilities

• Conduct holistic assessments that consider client’s physical, psychosocial, cultural, and environmental needs.
• Develop, implement and review individualised care plans with clients and their families.
• Build trusting relationships and advocate for clients’ rights, dignity, and preferences.
• Conduct regular check-ins, care reviews and site visits to monitor service quality and safety.
• Work with Care Coordinators, support workers and other providers to arrange appropriate services.
• Induct and guide support workers supporting non-clinical clients.
• Maintain accurate records and reporting in line with NDIS, aged care and organisational requirements.
• Build community relationships and help grow your client portfolio.
