Ready to make a meaningful difference in your community?
If you’re passionate about supporting people with disability and older Australians, and you’re looking for a role where you can build relationships and shape the care people receive, we’d love to hear from you.
Leora Healthcare is looking for a Care Partner to join our growing team across South East Melbourne. In this community-based role, you’ll work alongside clients, families, support workers and service providers to coordinate care that helps people live with greater independence, wellbeing and choice.
Our name, Leora, means light and that’s what we aim to bring: clarity, confidence and care when it matters most.
Leora Healthcare is a multi-award-winning provider delivering disability, aged care and community-based services across Australia. We provide person-centred care tailored to each individual’s needs and goals.
We’re proud of the culture we’ve built. Leora was named an Australian Financial Review Best Place to Work for five consecutive years (2021–2025), an HRD Employer of Choice (2023) and a Top 100 Global Inspiring Workplace (2023).
Our work is guided by our values: Clients First, Masterful Simplicity, Unleashing Potential and Do the Right Thing.
Starting in January 2027, you’ll manage a portfolio of clients across South East Melbourne, including NDIS participants and older Australians receiving aged care services, including Support at Home.
You’ll spend time in clients’ homes and local communities, getting to know their needs and coordinating the right support. You’ll be their point of contact, helping ensure your care is safe, personalised and aligned with their goals and funding requirements.
If you’re ready to do meaningful work, back your judgement and join a team that genuinely cares, we’d love to hear from you.
