Job summary

• Experience in community engagement and stakeholder networking.

Responsibilities

• Conduct holistic assessments of client needs.
• Develop and review individualized care plans with clients and families.
• Coordinate services with Care Coordinators, support workers and providers.
• Maintain accurate records in line with NDIS and aged care requirements.
• Build community relationships and grow client portfolio.
