Job summary

Catholic Healthcare Limited is seeking an experienced Care Partner for our Home & Community team in Port Macquarie. The role is highly autonomous, focusing on client independence, care planning, and building local networks to support community-based services.

You will drive stakeholder engagement, manage onboarding across multiple programs, and collaborate with clinical and operational teams to deliver high-quality client care with a person-centred approach.

Qualifications

• Strong stakeholder management skills and/or extensive case management experience.
• Minimum two years’ experience in Aged Care Industry within Home Care/Community services sector with strong understanding of case management and funding frameworks
• Strong time management and organisational skills with ability to prioritise
• Strong customer‑service orientation and ability to build rapport with diverse clients and supporters
• Intermediate to advanced skills in use of information systems and software packages such as Microsoft Office
• Demonstrated budgeting or financial management skills
• Current Driver's License and registered motor vehicle
• Experience in Sales/Marketing/Public speaking

Responsibilities

• Providing high-quality care management to support client independence and educate clients and families on program guidelines and contributions.
• Identifying client care needs and setting service expectations within budget guidelines
• Carrying out care planning aligned with CHL model of care with linked goals and services
• Completing care reviews, screening tools and risk assessments to identify services and referrals
• Managing new client sign-ups and onboarding across multiple programs
• Driving stakeholder engagement through local events and CHL-hosted activities
• Developing local networks by attending community meetings and building referral pipelines
• Acting as a trusted community liaison to strengthen CHL presence in Port Macquarie
• Using screening, assessment and risk dashboards to identify high-risk clients
• Handling incidents and complaints within required timeframes and resolving where possible
• Initiating case conferencing to support discharge planning and wellbeing interventions
• Coordinating with clinical and operational teams to plan interventions and review outcomes
• Overseeing service delivery per client agreements and funding parameters
• Maintaining accurate documentation in Client Relationship & Management Systems

• Pay rate – $48 per hour + superannuation +Km reimbursement
• Up to $18,550 tax free with salary packaging

We are seeking an experienced and values driven Care Partner to join our Home & Community team at Port Macquarie.

This is a highly autonomous position, ideal for someone who is self-driven, resourceful, and comfortable working independently.

The successful candidate will be confident in building local networks, driving community engagement, and managing priorities with limited on-site support

Day-to-day role looks like:

• Providing high-quality care management to support client independence, preferences whilst educating clients and their families on program guidelines, inclusions, exclusions and possible financial contributions
• Identifyingclient care needs, requirements and setting client service expectations within budget guidelines
• Undertaking relationship-centred care planning in accordance with CHL model of care and maintaining care plans that clearly link goals, risks, interventions and services
• Completing comprehensive care reviews, screeningtoolsand risk assessments promptly and accurately to identifyrequired services and referrals needed
• Managing new client sign-ups and onboarding across multiple programs, including Support at Home, CHSP, End of Life (EOL), AT-HM, and Restorative Care.
• Driving stakeholder engagement through presentations at local community events and the coordination of CHL-hosted events, both in person and online.
• Developing and maintaining strong local networks by attending community meetings, building referral pipelines, identifying business development opportunities, and collaborating with external providers.
• Acting as a trusted community liaison, strengthening CHL’s presence and fostering meaningful partnerships across the Port Macquarie region.
• Using screening,assessmentand risk dashboards to identifyhigh riskclients and prioritize timelyreviews

• Initiating case conferencing to support hospital, rehab or respite discharge planning for wellbeing interventions, serviceadjustmentsand timelyreferrals
• Communicating and coordinating with members of the clinical, allied health and operational teams toidentifyclient care needs, plan interventions, and review outcomes of care
• Managing the implementation of service delivery to clientsin accordance withclient agreements and funding parameters whilst maintaininghigh levelsof customer satisfaction
• Completing accurate and timely documentation and record keeping of client details in the relevant Client Relationship & Management Systems.

To be successful in this role you’llneed:
