Demonstrated experience in case management, budgeting and working with allied health professionals is essential. This role offers ongoing professional development and a collaborative team environment.

Qualifications

• Certificate IV in Community Services or related field required.
• EN/RN are welcome to apply.
• Current national police check or willingness to obtain.
• Working rights in Australia.

Responsibilities

• Onboard clients, conduct assessments and care planning.
• Review care plans and monitor changing needs.
• Coordinate services with clients, families and providers.
• Identify risks and escalate clinical concerns appropriately.
• Maintain documentation and comply with aged care requirements.
