Job summary

Australian Unity is seeking a Care Partner to guide customers through their care journeys with compassion in the Campbelltown area, with visits across Bowral and Moss Vale and two days work-from-home from the Campbelltown office.

You will develop holistic health plans, coordinate services with clinical teams and external providers, and ensure safe, person-centred care that supports aging well at home or in preferred locations.

Qualifications

  • Experience in community or aged care with case management.
  • Ability to coordinate multidisciplinary home-based care.
  • Strong communication and empathetic, confidential conversations.

Responsibilities

  • Build trusted relationships with customers and support them to achieve their goals at home.
  • Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
  • Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
  • Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
  • Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
  • Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
  • Promote safety, quality, compliance, and a culture of continuous improvement across all activities.

As a Care Partner, you'll guide customers through their care journey with compassion. This is a full time, hybrid role involving customer visits across the Southern Highlands areas of Bowral, Moss Vale etc, working from Campbelltown office and 2 days from home.

What You'll Bring
  • Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
  • Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
  • Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
  • Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning.
  • Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
  • Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers.
  • Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
