Job summary

Australian Unity is seeking a Care Partner to support customers on their care journey, coordinating services and developing person-centred care plans. This permanent full-time role is based at our Taree branch, with a hybrid work setup including client visits, branch days and WFH.

You will manage a customer portfolio, conduct assessments, coordinate multidisciplinary teams and ensure positive outcomes through ongoing reviews and proactive planning.

Qualifications

  • Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.
  • Strong customer focus with empathy and professionalism in sensitive conversations.
  • Solid communication skills and comfort with Microsoft Office and customer management systems.
  • Knowledge of funding streams, quality standards and discharge planning.

  • Build trusted relationships with customers and support them to achieve their goals at home.
  • Conduct onboarding assessments, develop care plans and coordinate multidisciplinary services.
  • Collaborate with clinical teams, families and external providers to deliver safe, person-centred care.
  • Support customers through care transitions, including hospital discharge and changes in health needs.
  • Identify unmet needs and optimise funding utilisation to enhance outcomes.
  • Maintain accurate customer records in enterprise systems and contribute to quality and compliance.

The Opportunity

As a Care Partner, you'll support customers throughout their care journey, helping them achieve their goals and remain living safely and independently at home or in a place of their choosing. Working closely with customers, families, clinical teams and external providers, you'll develop holistic care plans, coordinate services and ensure customers receive person-centred support.

This is a permanent full-time role based at our Taree branch.

About the role

As a Care Partner, you will manage an allocated customer portfolio and play a key role in delivering positive customer outcomes through proactive care planning, service coordination and ongoing reviews.

You will
  • Build trusted relationships with customers and support them to achieve their goals at home.
  • Conduct onboarding assessments, develop holistic care plans, coordinate multidisciplinary services and complete customer reviews as needs change.
  • Collaborate with clinical teams, families and external providers to deliver safe, person-centred care and achieve optimal customer outcomes.
  • Support customers through care transitions, including hospital discharge, changes in health needs and offboarding activities where required.
  • Identify unmet needs, optimise funding utilisation and support access to services that enhance customer outcomes.
  • Recognise and elevate customer risks, safeguarding concerns and changing care needs through appropriate pathways.
  • Maintain accurate customer records in enterprise systems and contribute to quality, compliance and continuous improvement activities.
  • Promote safety, quality and person-centred care across all aspects of service delivery.
What You'll Bring
  • Experience in community, aged care, disability or health services, including case management and coordinating multidisciplinary care.
  • Strong customer focus, with the ability to build trusted relationships and navigate sensitive conversations with empathy and professionalism.
  • Excellent communication skills and confidence using Microsoft Office and customer management systems.
  • Knowledge of funding streams, quality standards, audit requirements and care transitions, including discharge planning.
  • Strong decision-making, problem-solving, time management, and prioritisation skills in a fast-paced environment.
  • A collaborative mindset, with the ability to work effectively across multidisciplinary teams, families, clinicians and external providers.
  • Relevant qualifications in Community Services, Aged Care or a related discipline (Certificate IV or Diploma desirable).
