3Bridges Community Limited is seeking an experienced Care Partner to join our Aged Care team in Sydney, on a full-time fixed-term 18-month parental leave contract at Carss Park. You will manage caseloads, develop care plans and coordinate services to support clients’ independence, safety and wellbeing.

You will collaborate with frontline staff and internal teams, ensuring person-centred, responsive care aligns with funding requirements and quality standards.

Qualifikationen

• Tertiary qualifications in a Human Services discipline (e.g., Social Work, Psychology)
• Demonstrated case management experience in aged care or community services
• Strong communication, organisation and relationship-building skills
• Ability to manage care plans, client records and competing priorities
• Current unrestricted driver’s licence and relevant screening requirements

About the Role

Are you an experienced case manager who is passionate about helping older people live safely, independently and with dignity in their own homes?

3Bridges is looking for a Care Partner to join our Aged Care team on a full-time, fixed term (18 months) parental leave contract based at Carss Park.

In this role, you’ll manage an allocated caseload, building trusted relationships with clients, families and carers while developing and reviewing individual care plans, coordinating services and responding to changing client needs.

You’ll work collaboratively with frontline workers, the Rostering team, and other internal services to ensure clients receive responsive, person-centred support that promotes their independence, safety and wellbeing.

About You

You’re an experienced and compassionate case management professional who enjoys building strong relationships with clients and their families,along with a genuine commitment to person-centred care.

Essential Requirements

• Tertiary qualifications in a Human Services Discipline, i.e. Social Work/Psychology or similar area
• Demonstrated case management experience within aged care, community services or a related field
• Strong communication, organisation and relationship-building skills
• Ability to manage care plans, client records and competing priorities
• Current unrestricted driver’s licence
• Relevant employment screening and checks applicable to the role
