Job summary

Carrington is seeking a Care Partner to guide older clients through their care journey within the SaH and CHSP programs. You will deliver high-quality case management, coordinate services, and help clients stay independent at home across the Macarthur region.

This full-time role runs 8:00am–4:00pm, Monday to Friday, with on-site Grasmere office visits and client home visits. You will join a supportive team in a not-for-profit organisation committed to best practice and compassionate care.

Qualifications

• Qualification in Aged Care, Community Services, or related field.
• Knowledge of My Aged Care, SaH, HCP, CHSP programs.
• Experience in case management, care coordination, budgeting.
• Understanding of Strengthened Standards and Aged Care Act.
• Excellent communication and client-focused approach.
• Driver’s license and First Aid Certificate.

Responsibilities

• Deliver high-standard case management and service coordination.
• Collaborate with clients and representatives for an individual plan.
• Coordinate clinical care delivery with Clinical Care Partner.
• Manage client service budgets and unspent funds.
• Monitor outcomes and KPI achievement; drive improvements.

The Opportunity

As a Care Partner within our Carrington at Home service, you will guide older people through their care journey with compassion and expertise - within the Support at Home (SaH) program and the Commonwealth Home Support Program (CHSP) - supporting people to stay safe and independent at home as part of a multidisciplinary team. Enjoy this varied, full-time role working 8:00am - 4:00pm, Monday to Friday between our office Grasmere and cyclically visiting your clients across the Macarthur region.

Responsibilities:

• Delivering high standard case management & service coordination that is informative and responsive to individual goals and choices.
• Collaborating with clients and representatives to deliver an individual goal plan that supports assessed needs, goals and package resources.
• Working with the Clinical Care Partner to coordinate clinical care delivery within the quality framework.
• Ensuring that service delivery is within client's allocated service budget & managing unspent fund balances for the client.
• Monitoring service outcomes, ensuring continuous improvement and client satisfaction, and that key KPIs are met
• Supporting all clients with navigating My Aged Care and aged care services as required.
• Participating in an on-call roster (remotely) to ensure timely service provision.

About You

Essential Criteria:

1. Qualification in Aged Care, Community Services, or a related field; (or equivalent work experience).
2. Knowledge of My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs.
3. Demonstrated Case Management experience in coordinating care services, managing budgets, and working with diverse clients.
4. Have an understanding of the Strengthened Standards, Aged Care Act and related regulatory frameworks.
5. Excellent communication and interpersonal skills, with a compassionate and client-focused approach.
6. Driver’s License & First Aid Certificate.
7. Unrestricted right to work in Australia and ability to obtain a background check (National Criminal History Check / NDIS Worker Check).

• Aged Care experience - i.e. with My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs
