Job summary

Carrington at Home is seeking an organized Case Manager to deliver person-centred in-home care across the Macarthur region from Grasmere. You will coordinate Aged Care services, support independence, and work within a multidisciplinary team.

You will have qualifications in Aged Care or community services, knowledge of My Aged Care and CHSP/HCP programs, and a patient-centered approach. This is a full-time role with a local office base and client visits.

Qualifications

• Excellent communication and interpersonal skills to engage with clients and families.

Responsibilities

• Deliver high standard case management and service coordination.
• Collaborate with participants and representatives to deliver individual goal plans.
• Coordinate clinical care delivery with the Clinical Care Partner within quality framework.
• Ensure service delivery aligns with participant budgets and manages unspent funds.
• Monitor outcomes, drive continuous improvement and client satisfaction.
• Support participants with navigating My Aged Care and related services.
• Participate in an on-call roster to ensure timely service provision.

The Opportunity

As a Care Partner within our Carrington at Home service, you will guide older people through their care journey with compassion and expertise – across the Support at Home (SaH) program and the Commonwealth Home Support Program (CHSP) - supporting people to stay safe and independent at home as part of a multidisciplinary team. Enjoy this varied, full-time role working 8:00am - 4:00pm, Monday to Friday between our office Grasmere and cyclically visiting your clients across the Macarthur region.

Responsibilities

• Delivering high standard case management & service coordination that is informative and responsive to individual goals and choices.
• Collaborating with participants and representatives to deliver an individual goal plan that supports assessed needs, goals and package resources.
• Working with the Clinical Care Partner to coordinate clinical care delivery within the quality framework.
• Ensuring that service delivery is within participant allocated service budget & managing unspent fund balances for the participant.
• Monitoring service outcomes, ensuring continuous improvement and client satisfaction, and that key KPIs are met
• Supporting all participants with navigating My Aged Care and aged care services as required.
• Participating in an on-call roster (remotely) to ensure timely service provision.

About You

• Qualification in Aged Care, Community Services, or a related field; (or equivalent work experience).
• Experience and knowledge of My Aged Care, Support At Home (SaH), Home Care Packages (HCP) and/or Commonwealth Home Support (CHSP) Programs.
• Demonstrated experience in coordinating care services, managing budgets, and working with diverse clients.
• Have an understanding of the Strengthened Standards, Aged Care Act and related regulatory frameworks.
• Excellent communication and interpersonal skills, with a compassionate and client-focused approach.
• Driver’s License &First Aid Certificate.
• Unrestricted right to work in Australia and ability to obtain a background check (National Criminal History Check / NDIS Worker Check).
• Completion of a pre-employment assessment during the recruitment process.
