Job summary

BlueCare, part of the UnitingCare family in Australia, is seeking a Care Partner to oversee care planning and coordinate services for community-based clients. You will work with clinical teams to achieve optimal outcomes across aged care funding streams, with a focus on person-centred care and on-the-road client engagement.

You will develop and review care plans, manage budgets, liaise with clinical teams, and support clients to access necessary resources.

Qualifications

• Certificate IV in a Health or Community Service discipline is essential.
• Current Australian Driver’s Licence is essential.
• Roadworthy and insured vehicle required for work (Third Party Property minimum).
• Demonstrated understanding of client-centred, community-based care delivery.
• Ability to liaise with clinical and care teams to provide coordinated care.

Responsibilities

• Employment Status: Full Time - Max Term till January 2027
• Location: Redland Bay to Victoria Point & Surrounding Areas
• Regular travel to Macleay and Russell Islands required
• Remuneration:$53.55 + Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowances & Travel Reimbursement (KM’s) + Employee discounts

About the role:

As a Care Partner,your role is to ensure that our clients receive person-centred, high-quality care by overseeing all care planning and coordination of services. Working closely with the clinical teams to achieve the best care outcomes foreach client from their Aged Care Service funding through activities that include:

• Develop and review care plans in collaboration with the client and multidisciplinary care team
• Manage budgets and facilitate multidisciplinary care activities
• Liaise with clinical teams to monitor and respond to changing needs of complex community clients
• Continuously monitor, review and evaluate care plans, services and outcomes
• Assist clients in navigating the care system and linking them to necessary resources where required

Role requirements:

• CertificateIVin a Health or Community Service discipline -Essential

• Demonstrated ability to liaise with the clinical and care team and to provide co-ordinated care and exercise professional judgment within prescribed areas.
• Demonstrated experience and knowledge in Community Funding streams and programs
• Excellent communication and clinical problem-solving skills
• Confidence in managing your time effectively, including an ability to work autonomously.
