Job summary

City of Whittlesea is seeking a Care Partner to deliver person-centred, goal-directed care to live independently in the community. You will develop and review annual care plans, identify changes in health and wellbeing, and work with clients to meet their evolving needs in a supportive team.

This full-time fixed-term role sits in the Case Management team, collaborating with carers, family members, local agencies and council partners.

Responsibilities

  • Develop and recurring annual client care reviews and adjust plans as health and wellbeing changes.
  • Work with clients, carers, families and external agencies to meet needs.
  • Collaborate with internal teams and other council departments on care delivery.
  • Maintain flexible workflows and manage caseload discussions and reviews.
