Job summary

Avis Care Services in Brisbane is seeking a proactive Care Coordinator to oversee day-to-day participant supports, ensure service quality and maintain clear communication between participants, support workers and stakeholders.

This hands-on, fast-paced role requires strong organisational skills, effective scheduling, and the ability to balance competing priorities while supporting families and teams.

NDIS screening or willingness to obtain one, and full working rights in Australia are preferred.

Responsibilities

• Coordinate SIL clusters and assigned community participants.
• Engage with Participants and respond to changes in service needs.
• Collaborate with rostering to maintain staffing and continuity of care.
• Ensure shifts align with participant needs and manage scheduling commitments.
• Liaise with families, allied health professionals and stakeholders.
• Identify and address outstanding actions and provide guidance to support workers.
• Identify emerging risks and escalate clinical or operational concerns.
• Ensure participant documentation is accurate, current and compliant.

The Role:

• Work closely with rostering team to ensure shifts align with participant needs.
• Coordinate diaries and manage scheduling commitments.
• Liaise with families, allied health professionals and stakeholders.
• Identify and address outstanding actions
• Provide clear direction and guidance to support workers
• Identify emerging risks and escalation clinical or operational concerns appropriately.
• Ensure participant documentation is accurate, current and compliant.

About You:

• Experienced in disability or community services (desirable, but not essential).
• Proficient in Microsoft Office Suite and data entry.
• Current driver's license and access to a personal vehicle.
• Strong organisational skills with the ability to manage competing priorities.
• Confidence engaging with staff, participants and families.
• Ability to work independently, take initiative and follow tasks through to completion.
• High attention to detail and accountability for accurate documentation.
• Confidence working in a busy, sometimes unpredictable environment.
• NDIS Worker Screening Check or willingness to obtain one.
