AvisCare is seeking a proactive Care Coordinator to join our team in Canberra. This hands-on role ensures participant supports are delivered safely, consistently and in line with NDIS standards.

You will coordinate diaries, rostering, and communications with participants, families and support workers, while balancing needs with workforce sustainability. A calm, organised approach and experience in disability services will help you thrive in a 24/7 support environment.

Qualificações

• Experience in disability or community services (desirable, but not essential).
• Proficient in Microsoft Office Suite and data entry.
• Current driver's license and access to a personal vehicle.
• Calm under pressure in a 24/7 support environment.
• Comfortable managing complex, high‑intensity support environments.
• NDIS Worker Screening Check or willingness to obtain one.
• Full working rights in Australia.

Responsabilidades

• Coordinate SIL clusters and assigned community participants.
• Actively engage with Participants and respond to changes in service needs and communicate changes with support workers.
• Ensure support plans and documentation remain current.
• Balance participant support needs with workforce sustainability.
• Work closely with rostering team to ensure shifts align with participant needs.
• Coordinate diaries and manage scheduling commitments.
• Liaise with families, allied health professionals and stakeholders.
