Assistant In Nursing in Lake Haven, NSW seeks a compassionate Home Care Coordinator to plan, coordinate and monitor home care services for clients in their own homes. You will work with clients, families and support staff to ensure safe, person-centred care and timely service delivery.

The role requires a Certificate III in Aged Care or equivalent, current First Aid, a valid NSW driver’s licence and Police Checks, plus NDIS Worker Screening.

• Minimum Certificate III in Aged Care, Individual Support or equivalent (essential)

• Experience in Support at Home & relevant funding
• Excellent interpersonal skills and ability to build relationships with clients, families, and staff
• Sound understanding of the Aged Care Quality Standards and person-centred care principles

• Strong time management and multitasking ability

Home Care Coordinator

We're seeking a compassionate and proactive Home Care Coordinator to join our clients dedicated team and help ensure older adults and individuals with support needs can live independently and safely at home.

About the Role

As a Home Care Coordinator, you'll play a pivotal role in planning, coordinating, and monitoring care and support services for clients. Working closely with clients, families, and support staff, you'll ensure services are delivered with compassion, professionalism and efficiency.

Your day-to-day will include:

• Managing incoming referrals and conducting client assessments
