E4 People in Seven Hills is recruiting an Assistant Care & Services Coordinator to support the delivery of person-centred care for older clients in the Aged Care and Community Services sector. You will work with the Care & Services Coordinator to ensure care plans reflect each client's needs and independence.

You will coordinate staff, services and documentation, liaising with families, carers and health professionals.

Formación

• Certificate III or IV in Aged Care.
• Experience within Aged Care, Home Care or Community Care.
• Experience as a Care Partner.
• Understanding of aged care legislation, standards and guidelines.
• Strong communication and interpersonal skills.
• Excellent organisation and time-management skills.
• Ability to coordinate competing priorities.
• Practical, solutions-focused approach.
• Genuine passion for supporting older people.

We're recruiting an Assistant Care & Services Coordinator on behalf of an established Aged Care and Community Services organisation in the Seven Hills region.

This is a hands-on coordination role where you'll work closely with the Care & Services Coordinator to help ensure clients receive the right care and support based on their individual needs.

The opportunity

You'll play an important role in coordinating care and services for clients, while supporting the wider Home Care team.

• Assisting with the development of individual care plans


• Helping coordinate staffing and services around each client's assessed needs
