Job summary

Lumia Care Pty is seeking a proactive Care Coordinator to support the delivery of high-quality care across the Central Coast region. You will manage client information, care plans and service records, ensuring accuracy and compliance while coordinating onboarding and day-to-day service delivery.

Working closely with Care Partners and Team Leaders, you will prioritise tasks, deliver exceptional customer service and contribute to continuous improvement in our community-based services.

Qualifications

• Certificate IV or Diploma in Business Administration, Community Services, Health Administration or a related field.
• Minimum of 1 year experience in a coordination, administration or scheduling role, preferably within the health, aged care or community services sector.
• Experience using client management or rostering systems, with the ability to maintain accurate records in a fast-paced environment.

Responsibilities

• Coordinate care services to ensure clients receive timely, high-quality support.
• Maintain accurate client records, care plans and service documentation.
• Support workforce onboarding and ensure compliance requirements are met.
• Monitor service delivery and proactively resolve or escalating issues.
• Update client and workforce information across internal systems.
• Collaborate with Care Partners, Team Leaders and key stakeholders.
• Deliver exceptional customer service while managing competing priorities.
• Contribute to a culture of quality, compliance and continuous improvement.
