Job summary

Lumia Care Pty is seeking a dedicated Care Coordinator on the Central Coast to ensure high-quality care services, accurate records, and compliant documentation. You will work with Care Partners and Team Leaders to onboard clients and maintain service records.

The role emphasizes strong organizational skills, effective communication, and a proactive approach to issue resolution in a fast-paced health services environment.

Qualifications

• Minimum 1 year experience in coordination, administration or scheduling in health, aged care or community services.
• Certificate IV or Diploma in Business Administration, Community Services, Health Administration or a related field.
• Experience using client management or rostering systems, with the ability to maintain accurate records in a fast-paced environment.
• Highly organized with strong attention to detail and excellent communication skills.

Responsibilities

• Coordinate care services to ensure clients receive timely, high-quality support.
• Maintain accurate client records, care plans and service documentation.
• Support onboarding and ensure compliance requirements are met.
• Monitor service delivery and escalate issues when needed.
• Update client and workforce information across internal systems.
• Collaborate with Care Partners, Team Leaders and key stakeholders.
• Deliver exceptional customer service while managing competing priorities.
• Contribute to a culture of quality, compliance and continuous improvement.
