Job summary

Metro South Health is seeking an Aboriginal and Torres Strait Islander Health Worker - Chronic Disease to join a supportive multidisciplinary team. You will coordinate care, connect individuals with hospital and community services, and deliver culturally safe health education for chronic conditions.

You will work to empower communities, promote healthy lifestyles, and build partnerships that improve outcomes for Aboriginal and Torres Strait Islander people within Queensland's public health

Qualifications

• Culturally aware and compassionate health professional with strong communication and care coordination skills.
• Knowledge of Aboriginal and Torres Strait Islander cultures and community protocols.
• Clinical skills in chronic disease management (diabetes, respiratory, cardiac).
• Ability to work independently and with multidisciplinary teams.

Responsibilities

• Coordinate care and connect individuals and families with hospital, community health and social services.
• Deliver culturally appropriate health education and chronic disease support, including diabetes, respiratory and cardiac conditions.
• Build strong community partnerships to identify health priorities and improve health outcomes.
• Promote healthy lifestyle choices and empower individuals to manage their health and wellbeing.
