This full-time, onsite role involves service coordination, assessment, care planning, and review for non-clinical support programs to help older adults live independently at home.

The position requires a Certificate IV in Community Services or equivalent, strong MS Office skills, excellent communication, and a commitment to high-quality customer service.

Qualifications

• Certificate IV Community Services or equivalent demonstrated experience in a similar role.
• Strong understanding of the needs of clients from various funding programs (e.g. Home Care Packages, CHSP).
• Excellent computer skills with demonstrated experience in MS Office.
• Demonstrated commitment and experience in providing high quality customer service with excellent communication skills.
• Excellent organisational and time management skills and ability to work within limited timeframes.
• Police Check (Valid within the last 3 years).
