Job summary

Community Solutions is seeking a Behaviour Support Practitioner to join our team via flexible Work From Home arrangements in the Moreton Bay region. You will develop and support Positive Behaviour Support plans, conduct assessments, and work with families to promote safety, independence and choice.

You will collaborate with multidisciplinary professionals, provide training to support networks, and help build capability through coaching, with a strong emphasis on compassionate service delivery

Qualifications

• Tertiary qualification in a relevant field.
• Registration preferably as a Core Practitioner.
• Strong communications skills and the ability to build trusted relationships.
• Valid driver’s licence and willingness to drive; NDIS Worker Screening Check or ability to obtain.

Responsibilities

• Conduct functional behaviour assessments and develop evidence-based strategies.
• Complete Interim and Comprehensive Behaviour Support plans.
• Collaborate with families, support workers, and allied health professionals.
• Build the capacity of support teams through coaching and mentoring.
