Job summary

Optimum Health Solutions is seeking an experienced Behaviour Support Practitioner to join our growing multidisciplinary team across NSW and Tasmania, helping children, adults and families achieve meaningful outcomes. You will conduct assessments, develop Behaviour Support Plans and deliver evidence-based Positive Behaviour Support interventions in clinic, home, community and school environments.

A Core level NDIS registration is required and you will collaborate with families, carers and support

Qualifications

• Current NDIS Behaviour Support Practitioner registration (Core level or above).
• Experience as a Behaviour Support Practitioner or in a similar role.
• Strong understanding of Positive Behaviour Support frameworks.
• Experience conducting assessments and developing Behaviour Support Plans.
• Excellent communication and relationship-building skills.

About You

To be successful in this role, you will have:

• Current NDIS Behaviour Support Practitioner registration (Core level or above)
• Experience as a Behaviour Support Practitioner or in a similar behavioural support role
• Strong understanding of Positive Behaviour Support frameworks
• Experience conducting assessments and developing Behaviour Support Plans
• Excellent communication and relationship-building skills
• Ability to work independently while contributing to a multidisciplinary team
• A calm, patient and solution-focused approach
