Job summary

Real Therapy Solutions, part of the MedHealth Group, is seeking a qualified Behaviour Support Practitioner to support NDIS participants and families with assessment, planning and evidence‑based plans in a busy, person‑centred environment.

You’ll join a multidisciplinary team across Wollongong, Bella Vista, Campbelltown and Penrith, receive 1:1 supervision, mentoring and access to wellbeing and reward programs as part of MedHealth’s national network.

Qualifications

  • NDIS Registration as a Behaviour Support Practitioner is required.
  • Tertiary qualifications in a relevant allied health discipline.
  • Current driver's license and access to a vehicle is required.

Responsibilities

  • Support NDIS participants and families to identify needs via assessment and person-centred planning.
  • Own the assessment, development and delivery of evidence-based support plans.
  • Drive meaningful goals using a positive behaviour support framework and reduce restrictive practices.
  • Ensure plans reflect participants' goals, strengths and what matters most to them.
  • Build capability and understanding of those supporting participants in daily life.
  • Collaborate within a multidisciplinary team and mentor less experienced practitioners.

Job Description

In this role you will:

  • Support NDIS participants, along with their families and carers, to identify their needs through assessment and person‑centred planning
  • Own the assessment, development and delivery of evidence‑based support plans within a complex support environment
  • Drive meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices
  • Ensure plans reflect each participant’s goals, strengths and what matters most to them
  • Build the capability, confidence and understanding of those supporting participants in their day‑to‑day lives
  • Work collaboratively within a multidisciplinary team, including mentoring less experienced practitioners

Qualifications
  • NDIS Registration as a Behaviour Support Practitioner
  • Tertiary qualifications in a relevant allied health discipline or a related area
  • Current driver’s licence and own reliable vehicle
