Health and Aged Care Professionals is seeking a Behaviour Support Practitioner to support adolescents and adults with disability across the Central Coast and Lake Macquarie regions. You will deliver evidence-based Positive Behaviour Support in a well‑established multidisciplinary team, improving daily life and independence.

You will assess behaviour, create tailored plans, and collaborate with families and carers to implement strategies in home and community settings.

Qualificações

• Proficient in behavioural assessments and Positive Behaviour Support planning.
• Strong clinical reasoning and documentation skills.
• Ability to work collaboratively with families and allied health professionals.

Responsabilidades

• Conduct functional behaviour assessments and develop individualised Positive Behaviour Support Plans.
• Support participants with complex behaviours and psychosocial disabilities.
• Develop trauma‑informed strategies to promote independence and community participation.
• Monitor outcomes and adapt interventions to achieve progress.
• Collaborate with families, carers, support coordinators and other professionals.
• Provide guidance to support teams to ensure consistent implementation of strategies.
• Promote alternatives to restrictive practices under NDIS requirements.
