Job summary

1st Care Community Victoria – Airport West VIC is seeking an Assistant Care Partner to support older people to live safely and independently at home. You will work alongside a Care Partner to help clients access services that reflect their goals, preferences and cultural needs under Support at Home.

You will coordinate with care teams, help with care planning, and promote reablement, ensuring wellbeing and respectful, person-centred support tailored to each client’s needs.

Qualifications

• Experience in care coordination, case management, aged care assessment or community care planning.
• Understanding of person-centred care, reablement and planning around individual goals.
• Clear communication and ability to organise a caseload with guidance from a Care Partner.
• Current NDIS Worker Screening or National Police Check, as applicable, or willingness to obtain.
• A second language, particularly Greek, is highly regarded.

About you

• A relevant qualification in Aged Care, Community Services, Social Work, Case Management, Allied Health or a related field.
• Experience in care coordination, case management, aged care assessment or community care planning.
• An understanding of person-centred care, reablement and planning around individual goals.
• Clear communication, sound judgement and the ability to organise a caseload with guidance from a Care Partner.
• Current NDIS Worker Screening or National Police Check, as applicable, or willingness to obtain.
• A second language, particularly Greek, is highly regarded.
• Supportive team with flexible work arrangements.
• Training in the Support at Home reablement approach.
• Suitable applicants may progress to a Care Partner role as their skills and experience develop.
