Job summary

Department of Justice and Community Safety VIC – Melbourne VIC is seeking an Assessment and Transition Co-ordinator to support people through their prison journey and plan for release. You will assess needs, coordinate services with Corrections Victoria Reintegration Pathway, develop transition plans and parole applications, and work with providers to support people leaving prison.

Strong attention to records and reporting is required.

Qualifications

• Experience with risk or welfare assessment tools and making sound referrals.

• No mandatory qualification listed; tertiary qualification highly desirable.

• Ongoing, full-time role earning $81,496 - $98,955 per year, plus super.

What you will do

As Assessment and Transition Co-ordinator, you will support people through their prison journey.

You will assess needs, plan release and coordinate services with others.

You will:

• support the Corrections Victoria Reintegration Pathway and assess rehabilitation and reintegration needs
• develop transition plans and parole applications, with clear plans for release
• coordinate pre-release services and work with service providers to support people leaving prison
• work with people and teams to build practical plans for rehabilitation and community return
• maintain accurate records and data to support service planning, parole applications and program delivery.

About you

No mandatory qualification is listed for this role. A tertiary qualification in a suitable field is highly desirable.

You should have:

• knowledge of the transition and reintegration needs of people leaving prison.
• skills in using risk or welfare assessment tools and making sound referrals.
• communication skills that help you work sensitively with diverse groups, especially Aboriginal and Torres Strait Islander communities.
