Job summary

Centrecare is seeking an Assessment and Referral Officer at Level 5 in Australia. The role involves conducting assessments with people at risk of homelessness, making referrals to accommodation and service providers, and delivering face-to-face appointments for complex needs.

The position operates in a call room environment with high-volume client contact. Shifts are rostered—8.30am-4.30pm, 9am-5pm, 11.15am-7.15pm—with one Saturday/Sunday per month; 150 hours per four-week period.

Qualifications

• Bachelor's degree in Social Work, Psychology, Mental Health or related discipline.
• 3–5 years of experience in the social services sector.
• Strong assessment and interview skills.
• Excellent interpersonal and communication skills.
• High emotional intelligence and resilience.
• Available to work a 7-day roster including weekends.

Responsibilities

• Conduct assessments with individuals at risk of homelessness and provide referrals.
• Perform face-to-face appointments addressing complex issues.
• Build rapport with clients via telephone in a high-volume environment.
• Enter client notes daily into the client management system.
