Job summary

Knox City Council is seeking a compassionate Short Term Support Assessment and Referral Officer to join the Social and Inclusive team. You will be the first point of contact for residents seeking assistance, helping them navigate services, supports and community programs.

Based at Knox City Council Office in Wantirna South, part-time 0.8 EFT with flexible working while meeting operational needs. Band 5 classification with remuneration from $83,593.07 to $97,258.47 plus superannuation.

Qualifications

• Degree or diploma in community development / welfare or equivalent.
• Experience managing difficult telephone enquiries with empathy.
• Strong interpersonal and communication abilities with diverse groups.
• Experience undertaking initial assessments to identify needs.
• Understanding of disability and needs of people with disability, families and carers.
• Strong administrative, record-keeping and data management skills.

Responsibilities

• Receive, screen and respond to referrals and client enquiries.
• Conduct initial needs assessments and connect residents with supports and services.
• Provide information, advice and referral pathways for social, health, housing needs.
• Undertake initial risk screening and escalation as required.
• Maintain accurate client records and databases.
