Job summary

Tradewind Australia is partnering with a respected not-for-profit to recruit a compassionate Client Outcome Facilitator (AOD support worker) in Cairns. This role focuses on holistic, end-to-end case management, advocacy and practical support for clients on the AOD program.

You will work alongside people at different stages of their journey, listening, supporting, problem-solving and helping them move toward greater independence and wellbeing within clear policies and service agreements.

Qualifications

• Must have minimum Certificate III in Community Services or AOD related qualification.
• Experience in support work, case management or community services.
• Knowledge of client-centered practice, confidentiality and risk management.

Responsibilities

• Provide holistic, end-to-end case management and client support tailored to individual needs across the client journey.
• Conduct initial and ongoing client assessments including risk assessment and safety planning.
• Develop and review client-centered case management plans in line with policies.
• Deliver practical and emotional support that builds confidence and independence.
• Provide advocacy, information and referral support to help clients access services and strengthen community connections.
• Maintain accurate documentation and ensure a safe, compliant service environment.
