Job summary

Tallangatta Health Service is seeking a registered nurse to coordinate and facilitate consumer assessments for AN-ACC classifications and reclassifications as clinical conditions change in residents of our aged care facilities.

This is a permanent opportunity with flexibility for full-time or reduced week patterns. We welcome discussion of 3–4 day work weeks or other arrangements to meet service needs in a rural setting overlooking Lake Hume.

Qualifications

• Current registration as a Registered or Enrolled Nurse with AHPRA.
• Extensive knowledge and experience with provision of care within an aged care facility.
• Demonstrated ability to perform initial and ongoing assessments to support AN-ACC classifications and reclassification.
• Commitment to person-centred care.
• Well-developed interpersonal and communication skills.
• Strong time management and problem-solving abilities.
• Commitment to continuous improvement.
• Computer literacy with word-processing and data base programs.

Responsibilities

• Coordinate and facilitate consumer assessments to support AN-ACC classifications and reclassifications as clinical condition changes.
• Collaborate with multidisciplinary team to ensure timely assessments.
• Maintain accurate documentation and data entry for AN-ACC processes.

We are seeking a passionate Nurse to coordinate and facilitate consumer assessments to support AN-ACC classifications and requests for reclassification, as indicated by change in clinical condition of consumers within our Residential Aged Care facilities.
