CLINICAL CARE MANAGER

Role Purpose

The Clinical Care Manager is responsible for providing oversight and leadership to a team of nursing staff working at our facility and will provide support to the Facility Manager in ensuring the delivery of high quality person centred care, exceeding accreditation standards and maintaining continuous improvement activities and managing budgets.

The Clinical Care Manager is responsible for overseeing the incident management process, restrictive practice processes and support timely incident investigation in relation to the Serious Incident Response Scheme (SIRS) to support the Residents living in our specialist dementia facility.

Reports to the Facility Manager, with oversight and leadership provided by the Executive Manager Clinical Governance. The role forms part of the Clinical Governance Leadership Team of employer.

Technical

  • Provide clinical leadership to the team to ensure high quality care is provided to all residents living in the facility.
  • Regularly examine and review clinical incidents, audit data, clinical indicators and undertake clinical incident investigation, root cause analysis and reporting as required. Provide analysis of clinical incidents and trending and develop actions to support a cycle of continuous improvement. Escalate any areas of concern to the Facility Manager and Executive Manager Clinical Governance (EMCG).
  • Facilitate, in collaboration with the nursing team and Facility Manager, the monthly clinical incident and analysis report.
  • Provide clinical leadership and oversight of the High-Risk Resident Framework the facility to ensure Residents who are identified as having high impact high prevalent risks are supported and cared for using a multi-disciplinary team approach.
  • Ensure the legal and ethical compliance of all restrictive practice processes that are in line with the Aged Care Quality Standards. Ensure the process of informed consent is obtained from residents and or their legal representative, assessment, the development of individualised care plans and behaviours support plans are undertaken, and staff are provided support in understanding their obligations in minimising the use of restrictive practices. A key part of the role will be to support the Facility Manager in overseeing and maintaining the Restrictive Practice register and psychotropic register for employer.
  • Under the direction of the Facility Manager and broad direction from the EMCG lead infectious illness outbreak response required. Collate resident documentation and resident line listings for the period of the infectious outbreak working in collaboration with the Facility Manager.
  • Maintain the highest standards of documentation is to ensure accuracy and currency of care plans. Develop a schedule for care plan completion and monitor compliance.
  • Practice in accordance with the Nursing and Midwifery Board of Australia code of ethics, code of conduct and standards of practice required for registered nurses.
  • Ensure resident health is maintained in accordance with any advance care directives.
  • Develop, implement and ensure robust systems and processes to provide efficient service to delivery to residents and consumers.
  • Work collaboratively with the clinical team and the Facility Manager to ensure Residents are assessed and receive the accurate resource level required in line with the Australian National Aged Care Classification (AN ACC) funding model Involves completion of clinical assessments and participation in funding-related Consultancy reviews.   • Provide excellence in antimicrobial stewardship to improve resident outcomes.
  • Actively collect feedback from residents and their families to ensure timely and appropriate information, support and consultation regarding care needs.
  • Provide supervision and support to students undertaking nursing placements.
  • Participate in an afterhours on call roster. People or nurture talent and engage in succession planning, coaching and mentoring of the multi-disciplinary teams.
  • Identify and implement ways to creatively challenge and develop the team.
  • Assist direct reports to prioritise, delegate and effectively manage allocated workloads.
  • Provide support and guidance to the clinical team, including house companions, in providing holistic care to Residents living in the small house model.
  • Monitor and manage clinical supplies ordering processes for employer in collaboration with the Facility Manager. Establish and manage the imprest stock levels and ensure stock wastage is minimal.
  • Utilise data and analysis of industry trends and best practice products to inform decision-making for the purchase of clinical supplies.
  • Apply a risk management approach to ensure that any risks to persons in the work environment are effectively managed, including review of changes to work methods and practices.
  • Contribute to a safe workplace through adherence to all employer health, safety and wellbeing policies and procedures and a demonstrated commitment to improvement.
  • Ensure all health, safety and wellbeing concerns are reported and/or managed in a timely and appropriate manner, including hazards and incidents.
  • Actively promote a positive, friendly and happy approach in the workplace to enhance health and wellbeing.
  • Ensure appropriate levels of emotional support for consumers and employees.

Compliance

  • Maintain an up-to-date knowledge of legislative and regulatory compliance and ensure that requirements are achieved.
  • Compliance is reviewed and maintained through regular audit and review.
  • Ensure that the highest standards of privacy and confidentiality are maintained at all times.

ESSENTIAL

Technical Qualifications

  • Minimum of five years clinical experience with a focus on the provision of person-centred care and a commitment to the needs of older people. Knowledge, Skills and Experience
  • Advanced knowledge and experience in best practice dementia care, clinical care coordination, including chronic and complex health challenges, clinical care planning and Aged Care Standards.
  • Comprehensive understanding of AN-ACC and the ability to manage and coordinate AN-ACC assessments.
  • Well-developed communication and interpersonal skills including the capacity to work cooperatively within a team environment, effectively negotiate, resolve conflict and provide advice/guidance to staff.
  • Capacity to manage in a pressure environment and ability to exercise sound judgment in complex situations whilst working collaboratively in a multidisciplinary team.
  • Demonstrated high-level experience in supervising, coaching and leading a team and implementing continuous improvement programs.
  • Demonstrated ability to work autonomously.
  • Sound technical skills including and familiarity with the use of clinical systems.
  • Ability to participate in an on-call roster.

Licences/Other Requirements

  • Provision of a National Police certificate that meets the suitability requirements for unsupervised work in accordance with the Aged Care Act
  • Must hold a current drivers license
  • Must hold a current Registration to work with vulnerable people
  • Must be an Australian Resident or hold an Australian Working VISA
