Job summary

GV Health is seeking a Clinical Care Coordinator to support Ambulatory Aged Care Programs (TCP, REST, GEM In The Home) in the West Hume Region. The part-time role (64 hours per fortnight) coordinates care, advocates for clients and families, and works within an interdisciplinary team to deliver person-centred plans in acute, sub-acute and community settings.

Successful candidates will hold tertiary qualifications in nursing or allied health, current AHPRA registration where applicable, and

Qualifications

• Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) in the relevant discipline.
• For non-AHPRA registered disciplines, eligibility for membership of the relevant professional peak body must be demonstrated.
• Minimum of three years clinical experience across acute, sub-acute aged care and community sectors.
• Ability to case manage clients and coordinate services within a care coordination model.
• Ability to conduct comprehensive, person-centred assessments and develop goal-oriented care plans.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Provide care coordination, advocacy and support for AACP clients and their caregivers in the West Hume Region.
• Coordinate services to meet client needs within budget and regulatory requirements.
• Work within an interdisciplinary team to deliver integrated care across settings.

Education

Tertiary qualifications in Nursing or Allied Health
