GV Health in Greater Shepparton, VIC, is seeking a Social Worker for the Ambulatory Aged Care Program. The role is permanent part-time (64 hours per fortnight) within a multidisciplinary team to support older adults in the community and enhance functional outcomes.

The successful candidate will hold an approved Social Work qualification with AASW eligibility, demonstrate psychosocial assessment skills and a person-centred approach, and work collaboratively across GV Health and external services.

Qualifications

• An approved tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
• Demonstrated ability in assessment and intervention options for patients with diverse and complex psychosocial circumstances.
• Understanding of person-centred care in relation to adjustment to illness, disability or lifestyle changes.
• Ability to work independently and as part of an interdisciplinary team.
• Excellent interpersonal, written and verbal communication skills and the ability to maintain professional relationships.

Responsabilités

• Plan and implement high-quality social work interventions with a focus on continuous improvement and evaluation of outcomes.

About the department:

Ambulatory Aged Care Program (AACP) consists of 3 programs: Transition Care Program (TCP), Restorative Care Program (REST) and GEM In The Home (GEMITH).

The TCP provides short term support and active management for older people at the interface of the acute/sub-acute and residential aged care sectors. It is goal oriented, time limited and targets older people at the conclusion of a hospital episode who require more time and support in a non-hospital environment to complete their restorative process optimise their functional capacity and finalise and access their longer-term care arrangements.

REST provides the same support and management as TCP. The clients in restorative care are inpatients of GV Health while on this program. They can be younger clients (50+) and do not require an ACAS assessment to access the program.

GEMITH provides person focused care for people over 65, and operates within an integrated service delivery model utilising interdisciplinary team-based care with the aim to improve and maintain a person’s functional capacity and maximisetheir independence.

About the role:

This position will provide Social Work services to AACP clients, their families and significant others in a community setting. The Social Worker will function as an integral member of the Multidisciplinary team and be responsible for the planning and implementation of high quality Social Work interventions. The Social Worker will have a strong focus on continuous improvement, and the ongoing evaluation of the quality and outcomes of social work interventions. The Social Worker must communicate and interact effectively with relevant departments within GV Health and external services to ensure best practice outcomes.

Successful candidate will have:

• An approved tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
• Demonstrated skills in assessment and intervention options for patients with diverse and complex psychosocial circumstances
• Demonstrated understanding of person-centred care in relation to adjustment to illness, disability or lifestyle changes
• Demonstrated ability to work independently and as active member of an interdisciplinary team
• Excellent interpersonal, written and verbal communication skills, including the ability to initiate and maintain effective professional relationships with a wide range of internal departments, community agencies and services
