Job summary

GV Health in Victoria seeks a Social Worker for the Ambulatory Aged Care Programs to support clients in the community. You will plan and deliver high quality social work interventions within a multidisciplinary team, focusing on person-centred care and improving functional outcomes for older adults.

You will engage with families and carers, assess psychosocial needs, and collaborate with GV Health departments and external services to optimise care pathways.

Qualifications

• Approved tertiary qualification in Social Work and AASW eligibility.
• Experience with diverse psychosocial circumstances.
• Understanding of person-centred care in illness/disability.
• Ability to work independently within a multidisciplinary team.
• Strong interpersonal and written/verbal communication skills.

Responsibilities

• Provide Social Work services to AACP clients and families in community setting.
• Plan and implement high quality Social Work interventions.
• Contribute to continuous improvement and evaluation of outcomes.
• Coordinate with GV Health departments and external services for best practice outcomes.
