Sharp & Carter is seeking a compassionate Care Coordinator to support older Australians and those with disabilities in Sydney's Northern Beaches. You will assess clients, develop care plans, and coordinate home care while liaising with families and healthcare professionals.

The role offers part-time flexibility and potential progression to full-time management. We value strong communication, organisation, and relationship-building, with a Diploma in Nursing or Community Services preferred.

Qualifications

• Strong communication, organisation and relationship-building skills.
• Experience in aged care, disability support, home care or care coordination.
• Diploma of Nursing, Community Services or equivalent preferred.

Responsibilities

• Conduct client assessments and develop person-centred care plans.
• Coordinate home care services and manage ongoing client needs.
• Liaise with families, support workers and healthcare professionals.
• Monitor service delivery and respond to changes in client circumstances.
• Maintain accurate documentation and ensure quality care.
