Job summary

Senior Helpers is seeking an experienced Care Partner/Case Manager or Enrolled Nurse to join the Dee Why office on a part-time basis, delivering client-centred support to older Australians. You will assess clients, develop and review care plans, coordinate services under government frameworks, manage your local client portfolio, and liaise with our in-office EN/RN teams to ensure high-quality care.

Qualifications

• Proven background in care coordination/managing clients within Support at Home/ Home Care Package or NDIS programs.
• Strong communication abilities that help you build trust with client's family and work confidently with people from varied backgrounds.
• Highly organised, with excellent time management and strong attention to detail.
• Understanding of the Support at Home framework or training can be provided.
• Comfortable using technology, including digital care management platforms.
• New graduates EN's are encouraged to apply if past experience in aged care.

Responsibilities

• Conduct holistic client assessments and develop care plans that support needs, maximise client choice, well-being and independence.
• Conduct regular monitoring of services and care plan reviews to ensure optimal care and services are maintained to meet changing needs.
• Co-ordinate services and organise supports based on government compliant care plan for the client.
• Communicate effectively and build trust with clients and their family or representatives.
• Responsible for management of your own portfolio of clients for their care, Support at Home budgets and documentation. Portfolio will consist of a reasonable number of clients.
• Liaise and work closely with in-office EN/RN, scheduling and admin.
