Qualifications

• AHPRA Registration as a Registered Nurse is required.
• Experience in care planning, assessments and client reviews.
• Strong understanding of Home Care Packages and community care services.
• Excellent stakeholder engagement and relationship-building skills.
• Ability to balance clinical decision-making with compassionate client care.
• Strong documentation, organisational and time management skills.

Responsibilities

• Conduct comprehensive client assessments and reviews.
• Develop, implement and monitor individualised care plans.
• Coordinate supports and services to meet client needs.
• Maintain ongoing communication with clients, families and key stakeholders.
• Manage a portfolio of Home Care Package clients.
• Identify clinical risks and escalate where required.
• Support quality, compliance and documentation requirements.
• Collaborate closely with Community Nurses, Allied Health and Support Teams.
• Promote positive client outcomes and service excellence.
