Role Purpose

To provide respectful and person-centred support to a 55-year-old participant with mental health needs, assisting with daily living activities, community participation, and overall wellbeing while promoting independence and choice.

Key Duties

• Support the participant with daily living activities such as meal preparation and light household tasks
• Provide emotional support and encouragement in a calm and respectful manner
• Assist with community access, appointments, shopping, and social activities
• Support the participant to build confidence and independence
• Follow care plans, risk assessments, and support strategies
• Monitor wellbeing and report any concerns to the coordinator or manager
• Complete progress notes and timesheets accurately
