Job summary

LDK Villages is seeking an experienced Registered Nurse to deliver safe, evidence-based nursing care in our aged care settings. You will work within the RN scope, align with residents' care plans, and collaborate with a multidisciplinary team to optimize outcomes.

You will assess, monitor and respond to changes in residents' conditions, support medication management and provide wound, chronic disease and palliative care where appropriate.

Qualifications

• RN with minimum 3 years' experience.
• Current registration with AHPRA.
• Experience in aged care or community care desirable.
• Competence in modern nursing including medication management and documentation.
• Wound management, chronic disease management and palliative care experience desirable.
• Ability to use technology relevant to nursing/care delivery.
• Understanding of relevant aged care regulations, funding and accreditation.
• Ability to operate in a highly regulated environment.

Responsibilities

• Deliver evidence-based nursing care within RN scope and standards.
• Align care with residents' plans, dignity, independence and cultural safety.
• Conduct holistic assessments, monitor outcomes and evaluate interventions.
• Respond to condition changes and escalate per escalation pathways.
• Support safe medication management and administration.
• Provide wound care, chronic disease and palliative care.
• Collaborate with residents, families and multidisciplinary team.
• Coordinate with GPs, NPs, allied health and external services.
• Follow governance, escalation, incident management and safeguarding frameworks.
• Support staff development and escalation awareness.
• Mentor and guide less experienced staff and students.
