Job summary

VMCH is seeking a CHSP Service Coordinator to support older Australians to live independently at home. You will build genuine relationships with clients, coordinate services, conduct reviews, and navigate aged care pathways to ensure safe, person-centred care.

Qualifications

Experience in aged care, community services, disability services, health, or related field.
Excellent communication and relationship-building skills.
Ability to manage competing priorities and work autonomously.
Sound administrative skills and attention to detail.

Responsibilities

Build strong relationships with CHSP clients to support independence and wellbeing.
Coordinate services, develop care plans, and conduct annual reviews.
Monitor changing client needs and support transitions to additional aged care services.
Maintain accurate client records and ensure compliance with aged care standards.
Collaborate with internal teams to deliver person-centred outcomes.
Identify, escalate, and manage risks, incidents, concerns and complaints.
