Job summary

Goolburri Aboriginal Health Advancement Co. Ltd seeks an Aged Care Connector to help clients access essential aged care services within Mount Isa and surrounding communities. The role focuses on coordinating care, facilitating My Aged Care processes, and supporting culturally appropriate pathways.

You will work with older Australians to provide clear information, referrals, and community connections, ensuring high-quality, person-centered service delivery.

Responsibilities

• Deliver education sessions for older Australians, individually or in groups, about the My Aged Care process.
• Facilitate access and referrals to the My Aged Care system for Home Support or Home Care packages.
• Support older Australians through the Regional Assessment Service.
• Provide clear, accurate information on health management, services, and resources.
• Aim to delay or reduce hospital admissions for chronic conditions that can be managed at home or in the community.
• Strengthen culturally appropriate health pathways to meet the needs of Aboriginal and Torres Strait Islander people and non-indigenous communities.
• Promote meaningful social participation, connecting people with family, friends and community.
• Coordinate care among GPs, nurses, health workers, hospitals and community services via My Aged Care.

Purpose of the Position:

The Aged Care Connector reports to the Aged Care Manager. The role will contribute to ensure Goolburri clients receive the highest possible quality of care, by ensuring excellence in all aspects of our practice. Goolburri endeavors to ensure that clients receive the highest quality of service. We believe that the clients come first. Position Requirements:

• Education sessions for older Australians individually or group setting around process of My Aged Care system.
• Access/referrals to the My Aged Care System for older Australians needing Commonwealth Home Support Program or Home Care Package services.
• Support older Australians in the process with Regional Assessment Service.
• Providing necessary, appropriate, easy to understand and accurate information in health management, available services, and resource accessibility.
• Delaying or reducing the number of hospital admissions, including those for older people living with a chronic condition that can be managed at home or in the community
• Strengthening culturally appropriate health pathways to ensure primary care, community and at-home services meet the cultural needs of Aboriginal and Torres Strait Islander people and non-indigenous people
• Creating meaningful social participation that connects people to family, friends and community
• Improve the older Australians ability to coordinate the care they require, between:
  • GPs, Practice Nurses, health care workers, allied health and secondary health professionals
  • Hospital and Health services
  • Government and non-government funded community services (including services accessed via My Aged Care)
