Job summary

Barwon Health in Geelong is seeking a registered nurse (Div 1) with aged care experience to coordinate funding systems, monitor resident changes, and ensure compliant clinical documentation for regulatory obligations within the Residential Aged Care and Transition Care programs.

The role emphasizes audits, data collection for QIP, training compliance, and ongoing quality improvement, with a permanent part-time arrangement and a commitment to professional development and excellent records

Qualifications

• Experience in Aged Care is essential.
• AHPRA registration is required.
• NDIS Worker Screening Check is mandatory.
• Current Driver's Licence is required.
• Not subject to an Aged Care Banning Order.
• Proficient use of Word and Excel.

Responsibilities

• Coordinate aged care funding systems and assessments, monitor resident changes and care minutes, validate funding outcomes, and support accurate clinical documentation to maximise funding.
• Coordinate clinical and non-clinical audits, QIP data collection, care evaluations and mandatory training compliance, identifying risks and improvement opportunities.
• Monitor and report on restrictive practices, NDIS requirements, antimicrobial use and aged care regulatory obligations, maintaining registers, documentation and required reporting.
• Ensure accurate and compliant patient information management, support OHS requirements, maintain regulatory and organisational standards, and contribute to quality improvement and special projects.

About The Role

We are looking for a registered nurse with aged care experience who enjoys monitoring and managing aged care information to meet the needs of our residents and our regulatory obligations. The role of the care support coordinator will work within our team to provide support and direction in the areas of compliance and improvement initiatives across the Residential Aged Care and Transition Care Program services.
