Job summary

Ozcare is seeking an experienced Care Partnering Lead to provide leadership, coaching and operational support to a remote Care Partner team delivering client-focused care coordination for the Gladstone region.

You will drive service growth, ensure compliance with aged care standards, manage budgets and care plans, and develop teams while collaborating with clients, families and healthcare professionals. This role is based at the Rockhampton branch with a remote team.

Qualifications

• Demonstrated leadership experience in aged care or home care settings.
• Strong ability to coach, develop and support frontline teams.
• Extensive knowledge of Home Care Packages and client-centred care.
• Experience managing client budgets and care plans.
• Excellent communication with diverse stakeholders.
• Ability to balance competing priorities and meet deadlines.

Responsibilities

• Provide leadership and coaching to a remote Care Partner team servicing the Gladstone region.
• Ensure care coordination services meet standards and client expectations.
• Support assessment, planning, coordination and review of services for client independence and wellbeing.
• Oversee budgets, quotes and care plans for Home Care Packages.
• Build relationships with clients, families, referrers and stakeholders to drive outcomes.
• Monitor service quality and implement improvement initiatives.
