• Collaborate with SKCH, VincentCare and Ngwala to deliver wraparound support.
• Assist tenants to understand tenancy rights, responsibilities and agreements.
• Maintain accurate case notes and documentation per funding requirements.
• Participate in onsite meetings to align service delivery with partners.
